CHP works to transform community health by disrupting the ways we work together to solve complex issues. We don't attempt to solve these through direct services. Instead, we bring together diverse groups of people to change the conditions that hold a problem in place. By creating an environment of belonging, cultivating shared purpose, and facilitating a process of inclusive problem solving, CHP helps the community access a wide range of resources and ideas that lead to better health outcomes.
